Job Opportunity: Housing Case Manager
 
Join our team as we take action against poverty & work to create a more just and sustainable society. West CAP, an innovative, forward thinking organization, is recruiting a full-time Case Manager for our Homeless Prevention Program. Position will be based out of our Menomonie office. Responsibilities include providing case management to individuals & families experiencing homelessness; includes assessment, individualized service plan development, support, advocacy & referral.
 
Required qualifications: Bachelor’s degree in social work or related field; minimum of 2 years experience working with people experiencing poverty, mental illness, &/or substance use disorders; demonstrated experience with crisis intervention and case management; excellent written & oral communication skills; computer skills sufficient for data gathering, documentation, & reporting. Preferred qualifications: Experience working with individuals experiencing homelessness &/or trauma; HQS certification; experience with Housing First principles and knowledge of the services available. Must possess valid driver’s license, have reliable transportation, and deemed an insurable driver by our insurance provider prior to hire. Starting pay is $17/hr plus mileage reimbursement. Benefits include: health, dental, retirement, flex plan, & generous paid time off.
